Please file a new form for each non-consecutive date a program is to be recorded.

Requests submitted before 2:00 PM Central can be recorded the following school day.

Do not pick from both the Broadcast and Cable choices.

I certify that this request complies with the guidelines developed by Congress for educational use of off-the-air recordings and summarized here:
  1. A "broadcast program" is defined as one transmitted by television stations for reception by the general public without charge. These guidelines do not apply to pay TV.
  2. Off-the-air recordings may be used within ten (10) school days of the broadcast, once for instruction and once for reinforcement.
  3. Such recordings may be retained for 45 days for preview by teachers.
  4. After 45 days, or for pay TV program, we must request permission for use from the copyright holder.

No person shall reproduce material protected by copyright on District owned equipment without permission by those authorized to grant such permission.
